Dean Winchester is a complicated person. He has a lot of physiological issues. Most of these stem form abandonment issues his father, John, implanted in him. He has an extremely low view of his self worth and does not care for his own life. His first ideas are always to sacrifice himself and he has unhealthy codependent tendencies with his younger brother Sam. The Winchesters are known for their psychological problems, their excessive drinking, and their immediate need to sacrifice themselves, and anyone else, for their family. But Dean has to be the most self deprecating, self hating, self sacrificing, most abandoned person in the family. This stems from his father's constant reminders that Sam is his responsibility. He grew up knowing the monsters in his closet and under his bed are real. Since he was four years old, he was trained to kill monsters most adults couldn't even dream of. He never had a home or any stable relationships. He also has a extreme trust issues. Both his abandonment issues and his trust issues come from the fact that everyone he ever loved or trusted has left him or betrayed him at some point. This includes his brother and father, who were the only constant people in his life while growing up. His father also caused his self sacrificing and self deprecating nature by always criticizing him and treating him like a soldier. When the demon Azazel possessed his father, the only reason he could tell was because the demon was too nice. The demon was also the one that killed his mother, Mary, and he was too nice and understanding to be his father. His brother abandoned him so many times too. He ran away when they were young while in Dean's care. He abandoned Dean to leave the life of hunting and go to collage for four years without contacting him at all. Dean lost almost all trust in his brother when he was sent to purgatory and when he got out, without Sam's help, found out his brother left their friends to die, gave up hunting and never even looked for Dean. Dean sacrificed his soul and went to hell for his brother but he didn't even look for him. There's also a lot of proof that John abused Dean. In episode sixteen, in season five, we see the time Sam ran away under Dean's care and they hint that John abused him when he found out. It's also alluded to in several other episodes, including episode seven in season nine, where John left them for weeks with not enough food or money and Dean got caught stealing. When asked where he got hand shaped bruises on his arms he replied he got them from a werewolf but Dean hadn't known werewolves existed until he was in his twenty's. Sam also contributed to his abandonment and trust issues when he started drinking demon blood after his dying wish was to stop using his physic powers. It almost broke him when Sam chose the demon, Ruby, over his own brother. This is only including some of the problems that Dean's immediate family caused.
